Invoice and Contract template

Two questions:

1) Are there any places to download such templates that any would recommend.
2) If not what would be a good outline for both items above.
a. What do the clients need to furnish, like business description, pictures, ect
b. Color scheme
c. Layout
d. Pages desired
e. Keywords

And other items that right now do not come to mind.
