What About a Businesses Old Lapsed Customer / Client?

It often amazes me in the quest for offline glory by online offline marketing community the quest is always to acquire more and more new clients my question today is what about old lapsed clients?
It has been said many times before over many years that the biggest cost facing any business is the client acquisition cost, I know that traditional marking methods are changing but what has not changed is client loyalty it is an easier sell to someone who has bought before than to someone who has not.
Surely in the mix somewhere most be a mechanism to tap into one of the biggest assets of any business its existing and lapsed clients.
The question I ask today is how do you go about this do you send letters emails to existing customers with promotional offer and if this is done how do you charge for this service or indeed measure the success to charge for without a physical check of the order book.
