How do you expand your client's exposure online?

I have a few local clients that I provide SEO services for their business and I just wondered how you guys go about expanding their exposure online?

What I mean, is that I can get my client's website on Page 1 of Google for all of the obvious search terms relating to their business in the city they are in (e.g. plumber in birmingham etc) - but sometimes a smaller city they operate in means they only get 100-200 searches hitting their site per month.

I know I can target other towns/cities to increase exposure - but what if they only operate within their main city? How can I increase the number of targeted people seeing their website and expand their exposure online and increase profits?

    The population is smaller in suburbs than in major cities. If your client wants more business they need to expand the area they are willing to work.

    Yeah, I can see that expanding geographically is a cool way to increase leads - but what if this is not an option and they can only sell within a small radius of the city they live/work in?
