Looking For Rep Mgmt WSO

I am looking for a certain reputation management WSO that came out a while back.
Included in the WSO was a page you put on your site, that the client sends customers to, to leave their feedback.
It had 2 buttons... One green one for a good review, and a red one for a bad review.
The green one went to another page where it had links to different citation sources to leave a review at.
The red, negative review button, led to a contact form and then the complaint was transferred to the business owner.

Does anyone know the name of this WSO?

I know I have it... However, I have several WSO's and it would take me weeks to find it. And I would rather not go purchase everyone again, just to refund them since 1. I already had them and 2. they weren't the one I was looking for.

Any help appreciated.

    Reputation Management Business Builder. It's about setting a review portal for your client's customers but i never figured how to do this. If you know how, can you enlighten me.

      Thank you, you are a life saver. That is exactly what I was looking for.
      As far as a setup...mine will be on WP.
      It will be on a sub-domain.
      The home page will just have the two options
      Then, corresponding pages will have links to either a contact form or links to review sites.
