Choosing my geographical keywords

For google places eghem...Google + Local, I'm still somewhat new to the actual geographical ways of local as far as keywords are concerned. My question is,

Say I'm in pennsylvania, and wanted to serve 3-4 counties. Would I be best to try to rank for Northern Pennsylvania (will they distribute me throughout northern penn counties/cities?) or should I try to rank for the different counties (will they distribute me throughout the counties' cities?) or do I just break it all the way down and try to rank for the cities themselves (in which case is the most work).

Now I'll do the most work if it's necessary, but if google knows the difference, and distributes throughout the cities, then I won't have to. What's the recommendation here? Thanks in advance

    Google places is going to show your listing in the City your business is located in physically. The only way I know of to rank in other cities is to have multiple addresses.

    Wow really? Can anyone else confirm this? If that's the case then how can I help someone who has only 2,000 population in that city with google places? Seems like somewhat waist of time. Could still rank videos for him in other cities though
