Need A Suggestion For Website Design Questionnaire

I am going to build a wp plugin for my site's need, say a contact form with questionnaire, that provide potential client a list of questions about their identity and options about the website development packages those are provided by our site that clients can select and else, if the provided options doesn't meet their need, then they could specify a custom choices.

My question is, what are the information I need to collect from them and what packages options I should provide to them. I mean what form fields should be provided to get the best information as possible?

Anyone who could provide a detailed perfect/ amazing answer with explanation on how the plugin work will get a pro version this wp plugin for free after it has been finished. Because of you are the best part of this plugin development and as a thank you from me. A design is a plus, but it is not required.

    I'm confused. Are you trying to find out what questions to ask someone who wants a website, or are you looking for a design for your plugin? I have a questionnaire I use that you're welcome to if you want it.

      I am trying to figure out how a website development best questionnaire form for client should be. Including the questions and how it looks. I want to create the dynamic one. So, it will really helpful if there is also a design.
