AdWords Bid Adjustment Over 6 Month Report

Hi All,
I am trying to find or create a report that shows the bid adjustment at AdGroup level. The report will also need to show on a month by month basis how each AdGroup bids have increased / decreased whilst showing the impressions / clicks / ad rank / conversions in one report. Does anyone have this report or could point me where I can download, or an article on it, it would be greatly appreciated.
