Having trouble converting traffic

For the past few days I have been running a campaign on exoclick. I am promoting a survey site that pays out $1 per lead. I have gotten 830 clicks according to exoclick and 912 clicks according to my affiliate network. I have been able to get my CTR up to over 1.5%, but still can't seem to get any of it to convert. I have tried different CPC (.030, .010, and .20) with no help.
I am using 4 different creatives provided by the survey website (all 300x250).

I am only using one landing page (below), unfortunately the offer does not provide multiple.

Also I am not using a pre lander and I think this could be a huge help, but I wanted a second opinion before I invested any more time and money.
