I would not focus solely on the business owners name, regardless of whether you know their name if you are not compelling in the first 5 seconds they are not going to be interested anyway and pass...

I typically give 20% of my profit margin however it depends on who it is. If the person who has referred me is someone I know can bring me business time and time again I will increase this amount...

I agree, implement a help desk system and either provide the support yourself or if you have the funds available hire a virtual assistant. ODesk is good for this type of work.
