Facebook have a recovery option. You can contact their support. They need documentation to unblock your document. If everything goes well then we can recover the account again!

I agree with RobinInTexas. It's far easier to set up an HTML squeeze page from a template and FTP it to a separate directory in the root blog folder. You're then not restricted by any WP theme...
