Looking for script that asks 3 questions with a yes and no button

Hi All!


I am looking for a script to place on a Web page that allows me to define (3) questions...to be shown one at a time. Each question will have a 'yes' and 'no' button after it.


After the 3rd question is answered, a link will be provided below it.


Its like a 'survey' script that is used for promoting affiliate products or other offers online.

    If you're running Wordpres odds are there's a simple survey plugin that will do what you want for free.

    Do you need to save the data to a database or simply switching links at the end based on 3 options?

  • Yes....I usually use WP. No need to save any of the data.

    Not sure on your plugin.

    I've looked around and to be honest I can't find something as simple as what you need.

    If it doesn't matter that traffic leaves the first page and clicks to another page you could simply use internal links jumping to other internal pages.

    I've been using Bootstrap 4.0 and I can make a carousel do what you need, where there's no slider animation and I can make links inside each slider frame jump to another slide frame. So... each slide frame would have a question like a survey. Traffic wouldn't even know it's a slider, no slide animation, just loads the next question/content really fast. Unlimited slides and it's all built into Bootstrap.
  • I have started using the Quiz and Survey Master plugin. The base plugin is free but they have a lot of paid addons. From what little I know about it because I just started using it, I think it could do what you want but you may need to pay for one of the addons.

  • Got the script to work....simply updated the URL for the jquery to:

  • I guess you found your solution but Thrive Quiz Builder (for WordPress) can easily do what you're asking. It's basically a 3 question yes/no quiz.
