Do you have the comment prompt plugin?

This plugin prompts the user to leave a comment on the blog with a text box displayed in the post.

I checked out the original site at Comment Prompt/Reminder • Tim Nash “stuff” Blog which is down right now.

I need the plugin immediately.

Can anyone share a copy of the same if you've one or show me another plugin that is better than what this comment prompt plugin does?
