Anyone know how the G-Lock Optin Wordpress Plugin works?

specifically, I need to create a plain HTML form (not a wordpress page) that I can lightbox over my wordpress blog to initiate the optin process.

The form can post to whatever wordpress page required.

But I can't for the life of me figure out what fields to put on the form.

Does anyone know, or have an example they can point me at?
