need wordpress plugin for opt-in info

I'm looking for a free or lowcost wordpress plugin that will create a simple opt-in box to collect name, and e-mail address, and optionally phone number and a text box for user to optionally include a message. I'd like one that is easy to configure to send the information to an e-mail address.

    I just searched "opt in form" from the install widget admin area of my blog, there are several, I am installing one now, I will let you know what I find.

    I found one that works. install and drag widget and done. Called G lock double opt-in manager. millionairesdream.com | Reaching your Dreams, Collaborating with Millionaires .
    I guess if what I do works or not, the action of doing something gives me the opportunity to do something great.

    Still trying different things to make "what I want".

      Thanks. That one doesn't sound quite right for my current use. This one is really for the customers to tell us a bit about themselves so we may contact them manually by phone or email to set up an appointment.
