Okay, the form is intended to be run on PHP, because on html file, there is PHP code and i can see it from here (medical123.co.uk)

Just delete the index.html file and leave the index.php file.

All done I have also managed to make the form look as I wanted was a problem in the form.css file, teach make for being such an amateur
