What kind of script is this called?

Hi there expert programmers!

I've been testing out certain scripts and wordpress plugins and i came across one script that I'm not sure about. Initially I thought it was some kind of database script, but I could not find anything similar to it while Googling for it.

This is the page which contains the script:
Netmaid - The largest maid information hub in Singapore

Does anyone have a clue to the actual term of this kind of script?

    you mean all the checkboxes, dropdowns etc?

    That's a custom script, linked to their DB - I doubt you'll find something "out of the box" like that.

    Try some of the "forms" solution that are out there for wordpress, some of them should cover you from the input side - the db selection etc., that depends on your current DB structure etc...

    is somewhat limited to contact forms and might not be sufficient depending on what you need, but have a look, maybe it'll lead you in the right direction.
