Brief: Need a bit of code which adds extra fields into the paypal buy now button

It's for an SEO service so I need just a few simple blocks. eg:


URL [you write here]
Keyword 1 [you write here]
Keyword 2 [you write here]
Keyword 3 [you write here]
etc

The fields need to be filled out before you can press the buy now button.
