Please I need to have add an autoresponder form inserted inside into an image sort of what Dustin has here at:

    Hey Ron,

    He is using an image as a background image, you can set it via CSS and on top of it you add the autoresponder code.

    Inside the code from that site you can find div with id="trngopt_topblog" that using background image and inside that div you'll find the autoresponder code.
