Pop Up Email Capture Form?

PROGRAMMING

On my website I want to create a pop up that has my opt in form. I want to be able to use the html code from my autoresponder, and have that form pop up each time.

Is there a non complicated way to do this? I know HTML and CSS, but I'm clueless about javascript and PHP so I'm really confused how to do this.

Put the form in a div like so:

<div id='MyForm' style='display:none; position:absolute; top:100px; left:50%; margin-left:-250px; width:500px;'>

</div>

make a button or something to trigger the pop up add an onclick to it:
onclick="$('#MyForm').css('display','block');

** This example uses JQUERY for the onclick.
