php form recommendation

Anybody know of a FREE feedback form where I can have name,email,phone,drop down fields, radio buttons, that is FREE for $10-$20.

also it must be extremely secure, very advanced for anti-spam, and php

needed for formmail and lead gen.

(NOT Tectite and NOT for a plugin for Wordpress site. Site is not wordpress driven. It is php based

EZ to set-up, no MySql requirements either.
