What to do With Existing Backlinks After URL Rewrite

I have a php driven site and want to make the url's more friendly..
/mypage/mycontent rather than index.php?page=21.
What I would like to know is how best to handle existing links back to my site. They will have the old url. Should I redirect them to the new url, or should I leave them and just place a conical reference to the new url.. Or both.. I am uncertain what is best.
Any suggestions?

    While the canonical link element has its benefits, Matt Cutts, the head of Google's webspam team, has claimed that the search engine prefers the use of 301 redirects. Cutts claims the preference for redirects is because Google's spiders can choose to ignore a canonical link element if they feel it is more beneficial to do so.
