Not always an easy question to answer. Mostly people worry about this when using software. Because I prefer to build high quality links by hand I am less concerned about this. However, I would suggest you always build a good number of naked url links to your site first, before starting to drop exact match etc. Even then, you don't really want anything more than 5% exact on your site.

Quality over quantity is the way to go though with any of this

Keyword density does not matter, let me share with you what does.

1: Keyword needs to be used in your Title Tag and permalink.
2: Keyword needs to be used as a H1 tag at the beginning of your content.
3: Keyword needs to appear in the first 100 words of your content after the H1 Tag.
4: Every 300 words after the start of your content TRY to use your keyword IF it makes sense. It doesnt need to be exact match either. For example, if your keyword is "Hot Dogs" you could say something like, "Hot Dogs with mustard".
5: Make sure any images (You need to have images) have alt tags with the keyword in them.

The question was not about keyword density, but yes keyword density does not matter.

You are talking about on-page SEO. I'm talking about off-page SEO. A lot of SEOers recommend to use different density of KWs. It depends on the site (long term or short term site).

    My anchor-text is approximately 17.874% (+/- 0.001).

I used to think about those ages ago when I used to not be able to rank anything.

These days, I link with whatever phrase makes most sense in the context and I don't care whether I used it before or not.

The best rule which is also future-proof is to write naturally. I know it sounds lame but google is getting better at picking up unnatural keyword usage.
