I paid for an opt-in form but they coded with H2-H4 tags. Could this hurt my on-page SEO?

I decided to add some CTAs to my page, and one of the ways was through an opt-in form above the fold.
I'm looking at the code of the opt-in form and I noticed he used H2-h4 tags in it.

But I already used proper H2-H4 tags for on-page SEO.
Question: Will this negatively effect my SEO? I would think so, but im not sure.
Should I tell the designer to make it without H tags? or is that not possible to do?
