Is Google Places no longer picking up citations?

Hi Everyone,
I just brought this up with Brad Callen, and thought I'd ask the rest of you.
I have noticed that my the citations I have been laying out for my clients have not been picked up the last few months, in spite following all the right procedures.
I'm not sure why Google is not connecting the dots, but I'd love to see what the proper citation is if I'm not currently doing it correctly.
