Google page #2 in SE and #23 in the US?

Hi Guys,
I have a site called achillestendonitis.n.nu with the keword Achilles Tendonitis. There are approximately 40 000 searches per month for this keyword.
After some SEO work I was very happy to make it to Google page number 1, position seven.
The good position did not generate much traffic so I started to do some investigations. A friend at the forum gave me a tip on for an add on for Mozilla Firefox called Google Global.
I am at page 23 for Google in the US? Why such a gigantic difference? My page number one result is for Sweden, but all my articles and all my SEO work is in english?
Do you think it is about the ".n.nu" part of my domain name? It is commonly used in Sweden instead of .se because .nu is swedish for now.
Any good theories? Should I get myself a .com domain instead?
