    1) Remove the files from your server and ensure they return a valid 404 HTTP header

    2) If your site overall gets de-indexed, submit a reconsideration request form on Google

Just remove the file or mod rewrite it and leave it just there, it will soon de-index it naturally.

First, are you sure you were "attacked"?

That does not look like any hack job, just php code for
        some type of form input.

Why would you not want your directories indexed?

Those are not separate pages. Everything after a "?" in
a url is not a separate webpage. It's just passing code
and is actually ignored by the browser. Try it yourself.

You don't have 3380 different webpages.

        Go to warriorforum.com/?=nothing
        and see what site loads.

I think you are confusing "indexing" with SERPs. Google
isn't going to show anything in SERPs unless it's related to
the search. Rarely do you have to worry.

You could just delete the directory. But then, if you need the
        directory...why wouldn't google index a directory, assuming it's
        a normal directory?

Google will index just about everything...no big deal.
