My question is when we are trying to maintain a good search engine friendly keyword density we have to make sure the keyword density is 2% - 5%.

But to measure keyword density should we count keywords used in Title, Domain etc. outside of the <body></body> tags too? Or we just need to count keywords within the body tags?

You should count only the keywords present in the body content excluding title tags, domain etc

You're right, seoquake takes into account title tag, when calculating keyword density.

I wouldn't look into keyword density too much, as long as you have it scattered around a few times. The most important thing is user experience and article readability.

Keyword in title, domain, description, etc. is not just optimized for SEO density which is also an aspect of SEO like keyword included in H1 tags. I think we should try to be best on everything about SEO and this is a must.
