You have to imagine, you're not the only person who knows how to building links, your competition is constantly putting effort into achieving good SERP ranking. I monitor my keywords with a software and Google is dancing so much these days that it it landed a user with my site on #5 it may land them on #23 tomorrow and vice versa is also true. With the exception of handful of keywords, too many changes in algorithms this year. From 2001 to 2011 it was smooth sailing but I have been sea-sick all this year.

Very broad question. On some keywords that are extremely easy to rank for-you could stay ranked 1 for a year or longer. However, competitive keywords need constant care. You have to keep feeding the landing page links, fixing the on page, etc.
