How Do You Find Leading vs. Trailing Keyword Combos?

Let's say you were trying to find keywords related to "Counseling". When I punch that into a keyword scrape suggestion tool I get:

counselor
counselors title
counselors title dc
counselor salary
counselor troi
counselor jobs
counselors in fredericksburg va
counselor in training maryland
counselor job description
counselors in maryland


These combos are all trailing (they trial the main keyword) so how would I go about finding leading combo words? As in:

family counseling
marriage counseling
diabetes counseling
church counseling
pediatric counseling
etc.

      Yukon...I don't think I communicated what I was trying to achieve very well. See, I don't actually know what the leading words are. I need a tool where I can punch in "counseling" and have it spit out family counseling, genetic counseling, pediatric counseling, etc.
