My first post after Penguin, how should I do it?

I'm uploading my first post to my site after being hit by Penguin last week. I'm not sure how I should it.Please help me with these issues:
1. Should I include my keyword in the title of the post?
2. Should I include my keyword in H2 and the other Htags
3. Should I include my keyword in the alt image ?
4. I use platinum seo pack. Should I optimize keyword and description tags provided by Platinum or leave them blank?
5. How many times should I mention my keyword in a 1000 words long post?

    Well, I've been doing it this way and had no problems with the update.

    H1 title tag with main keyword

    content (bolded main kw also in first sentence)

    Image with secondary keyword alt tag

    H2 with "main keyword": "variation of secondary keyword if possible"

    content

    H3 with secondary keywords

    content (main kw in last sentence)

    If really needed, i use more h3 tags with secondary keywords

    As for keyword density, I usually just write my article and then see where i can insert the main keyword without it looking too odd or misplaced (strive for quality content), I never go higher than 2% main kw density.
    as for secondary keywords... they just naturally pop up when I start writing, when you write about a subject, look at all the possible keywords you can use and keep them in the back of your mind when writing the article.

    I still use meta description with my main kw once. Meta tags include all the keywords I used in the article (normally I use about 4 to 6 tags).

    write the post so it sounds natural and informative. Make sure your visitors will enjoy it before Google does.

    Here is what I am doing, without any problems, in fact I have benefited greatly from the updates.

    H1 Main keyword
    H2 Secondary keyword
    H3 Third keyword

    Use main keyword in first and last sentence of your post. Bold your first use of your main keyword.

    Now just write about your keywords and forget about density, just engage your readers.

    Posts to be at at least 1000 words, break up your posts with images, bullets, charts, whatever it takes to make the user experience a good one. Write or have written your posts to benefit yours readers and not your pocket and you can't go wrong.

    Backlink your keywords using layers of links.

    Bookmark your pages with authority sites in your niche, RSS your bookmarked pages, backlink your RSS feeds. Import your list of pages to other bookmarking sites..

    You can use your targeted keywords in title and body part but dont try to run for keyword density factor. Write an article naturally and use your keyword genuinely.

      Just write it naturally, and let it works its magic. Of course you should still do proper on-site optimization. I wouldn't recommend shooting for a certain keyword density either, just write naturally.

    Do not include many keywords, just make the flow of your content natural..If you are inserting keywords, insert it on the first, middle and last part of your content..

    To be honest I wouldn't pander to Google when in fact what you should be doing is posting good quality Original stuff!

    If you're writing just specifically to get ranked then people will realise that when reading and might turn away.

    use only one H1 tag per page, avoid advertisement on top fold .. get backlinks from relevant content .. This should help you recovering from Penguin update.
