I have a few question about Google analytics. See the image about the statistic of one single page below. All traffic came from PPC.

1. United States has 15 pageviews with Avg Time on Page is 00:00:00. What does this mean? They did not open the page for one single second?
2. India have 14 unique pageviews, and 42 pageviews. What did the 14 unique person doing in my page? Refreshing the page? Or maybe they click the ads several times? All traffic came from PPC. Does this means the PPC I'm using has click fraud?
3. What can you conclude from this statistics?
