You've already got the answer. Either use the canonical tag, or 301 redirect the old URL format to the new URLs. Either way is going to take some work, possibly a little programming, but sometimes doing things the right way takes work.

You can use canonical tag [<link rel="canonical" href="http://www.example.com/page.html" />] or use 301 redirection via .htacess file.
