Should we build a new site and start again?

I have been approached by a cosmetic surgeon to help him get his site ranking back onto page one of Google.

The site was ranking highly and getting great traffic until January this year. Then the traffic dropped like a stone over a few weeks, to virtually nothing. He's no longer ranking anywhere for any of his main keywords.

He's had an SEO company working on it, who were obviously getting success initially with methods that Google didn't like. Now it appears the site has been blacklisted.

The site has about 150 pages of content, all great keyword rich content.

But the question is, should I try to figure out why it's been blacklisted, or should I just start again with a new site and do everything "by the book". I'm pretty familiar with the basics of on-page SEO.

What would you do? Would you try to resurrect the current site, or start from fresh? What are the pros and cons?

    Is your website still in Google? Because you said "He's no longer ranking anywhere for any of his main keywords."

    What about the others (like the domain name?)

    If the whole website is BANNED, then, yes... You should start away... If it is only a few keywords, this is temporarily and you could achieve again the ranking, just look for the NEGATIVE backlink!

    I think it would come down to whether or not the site is entrenched in a lot of publications.

    For example if the site has good links from medical journals and legit publications, or is already an established "brand name" of some sort, then you may want to work to salvage it (disavow tools, more quality links).

    If the site was purely propped up on crappy SEO links and the domain name fairly disposable, then I would say to start over.

    What are you going to do have the cosmetic surgeon change his name, the name of his practice, the name of his site, etc...?

    Doesn't sound like your talking about a disposable Amazon affiliate site. Most of the advice you'll get around here will be based on disposable affiliate sites, which isn't appropriate for a real offline business.

      His current domain name is based around the doctor's own name and it does have links from quite a few genuine directory sites etc. So that's a plus for keeping it.

      But we could find another domain name that still works and sounds professional. And at least we would know everything is strictly legit in Google's eyes.

    One thing that I would suggest, if he is not doing this already, is creating a Google+ Local listing for his business. Therefore, if you do end up starting over with a new website, you will be able to get his practice ranked prominently again with Google+ Local.

    The catch is that with Google+ Local, everything has to be done a specific way in a specific order in order to gain a page one listing.
