What determines if Google Authorship shows up in SERPs

As the topic states, what factors determine if your google authorship image shows up in the SERPs? I have several pages from my website that display my G+ image in serps correctly, and several that do not. I have confirmed that all have google authorship attributed to the page correctly with the rich snippet tool.

There doesnt seem to be any method to the madness yet, it seems this concept may still nebulous as google itself figures out how to incorporate this feature.

Have any of you noticed any factors that help ensure your authorship image shows up in SERPs?

It does seem pretty random as of now. I would probably just wait it out to see if more pages begin to show up.

As far as I have seen, all my websites search results are displaying the claimed author's G+ images. But I have heard some people have the same issues like yours. Seems weird.

For example, see the image below. I wonder if this is accurate information, because I do have a picture of my wife and I in the profile image. Perhaps that is why we are not showing up in the SERPs everytime.

I see that if I display star ratings in my search results, my picture doesn't show with the stars but my name is displayed. I'm hoping that this means I still get the authorship bump in SERPs for these posts.
