I use Affiliates Enterprise by ITThinx. MagicAffiliates used to be pretty good, but expensive. WPMUDev's Affiliates is also good. Depends on what your needs are.

You could use Visual Form Builder Pro or Gravity Forms. Each has a PayPal plugin. I've created this with these tools many times.

There are a ton of them on Codecanyon. Just look under "CSS," then "Pricing Tables." There are about 50-60 of them. There's also a plugin or two on Wordpress.org. Just look for pricing table.
