Once you get to the front page you normally have title, meta, keyword, content, etc all in a good place. It normally comes down to anchor text links at that point.

I am in the top 3 results for a 40K search volume (exact) keyword. Webmaster tools says ive only done 5400 impressions for that same keyword.
