In my experience CPM is very often defined by the type traffic. It all depends on what countries you get your impressions from and what type of site you are running. I usually get around $0.2-0.3 CPM...

CPVlab is decent, worked with it. Epom's ad server is another great alternative for tracking, mostly because it's super easy to set up tracking there.

To have a faster approvement you just need to give a call to company. Then... website isn't a must as was already mentioned, but you'd better have one. Really, sooner or later you'll have to get one....
