Depends on the template, but you should be able to just replace the function that currently enrolls the subscriber into the AR with a simple mailto that directs to your email.

Can't offer any help with the software, which does seem to have disappeared. I would suggest that you use openvbx, you should be able to accomplish the same thing.

Why not gather both? Let the customer decide how they want to be contacted. Worst thing that happens is you get the same customer on both lists, and they have two reasons to come in.
