Yep disavow them if they dont have any good content on them. If they have good content, with dofollow links, leave them, as it is still possible they are helping rankings.

Depends on many things. Mostly how much juice does the page have (backlinks coming in/PR) and how many outbound links are on the page. The key is to go after high PR pages with low outbound links.
