For all of them as each has a DIFFERENT weight! Of course you should do it in the right way and not just do keyword stuffing! ;)

There's no official limit, my guess this changes by niche and specific keyword SERPs but I could be wrong.

I recommend sticking around 10-15% at the most, and only adding more optimized anchors as...
