Ibuildapp is good because it has a a lot of templates with ratings. The problem is a template cannot be changed without data loss.
I have also used ezmobilesitegenerator with no problems.

Anywhere from $200-500 depending on difficulty and client.
I have heard of people that have also done elaborate 7-8 pages...
