CTR means Click Through Rate. And nobody can say what will be CTR. It can only be predicted that if your ranking improves the CTR will improve and if it fells then along with it CTR will also fell.

It should be added to every URL that is duplicate (or substantially duplicate) content and point to the original source URL.

You can also add it to the original source URL and point to itself.

ASHX is what would be used for an ASP (Windows) based website. XML is what would be used for a PHP (Linux) based website. Both do the same thing, it comes down to what type of web hosting you use.
