Also missing is information from Google Trends to see if searches, overall, for your niche and targeted kw phrases has declined over that period of time (it happens!).

Those percentage figures don't really tell us much in themselves. In fact, if your overall visitor numbers dropped by 20%, you've actually seen an increase in real numbers on your non-organic...

I don't see any reason why this would affect your rankings.

When you make a Google search for "401k", there are a mix of results from some websites that are using parentheses and some without....

404 errors do not negatively affect website rankings unless your whole site is 404 errors and you are linking to those defunct articles. It's almost impossible for a site not to have 404 errors.
