Yes there is an option to HTML format and then there will be headers.
Its also possible to add own instructions to the prompt templates, so you can instruct the AI to add special formating.

It does not need a ChatGPT subscription, but it needs an OpenAI API account. If your API account is authorized for GPT4 then it can be used yes.
