Yes, there is a per copy charge - VERY minimal, like 2 or 3 dollars (plus shipping).
The $40 is to publish to expanded network - has nothing to do with how many copies you order.

There is no cost unless yo choose an expanded distribution option, which I believe is around 40 bucks.
