Number 3. When people go to type in your name, they will often forget that there was a hyphen in it. Also, .co is not as widely known as .com. Plus number 3 just sounds better than the other ones.

Use both if you are able to. Amazon's good because their ads are everywhere and Clickbank's good because you could potentially have tens of hundreds or more affiliates promoting only your product.
