Social Media Marketing- FB ad not converting

I have posted an ad on facebook. The ad is of CPA offer from Peerfly.
This is the ad preview:

It got 3451 reaches with 18 clicks spending $10.24.

After clicking the ad, the audience is taken to a landing page where there is only one call to action button:

It means only 18 people have reached on this page. After clicking on continue, audience is taken to the real offer page where they will find a form to fill in order to be converted/signed up. The offer page is:
http://imgur.com/a/UDPhn
Only 2 people have reached on this page and no one signed up using the form.
Only 2 people out of 3451 people.
Please help me find out how to optimize it to make it a high converting ad. Thanks.

    Has Corey approved your ad, mate? Using the term "free" (or any suggestion of it) is a no-no in PF affiliate promotions. If Corey has, no worries.

    About the CTR. I'd need to know more, but . . .

    2. The ad is white noise.
    About 2. Your audience has seen that ad a million times or more. Well, ads just like it. You need to achieve 2 things. First, grab attention. Second, whack them at your lander.

      Ans. There isn't any restriction on this offer which stops me to use the word FREE. The offer is an approved offer.

      Ans. Since the offer is of making money from taking surveys, I have targeted the people who are interested in making money online, home based jobs, paid surveys etc.

    It seems that your targeting settings is too broad.
    Don't just add the interests but also use the function " MUST ALSO" to narrow your audience and increase the quality.
    You said you are targeting interests like making money online, home based jobs, paid surveys ...etc. There are unlimited ways to make money online, and home based job is not necessary to be through "paid survey". You need to be more precise. You can target competitors who provide paid surveys opportunities.

    See the following example of how you can use the function "Must Also" and targeting the competitors:

    Interest: Paid survey
    then click narrow audience
    MUST ALSO: CashCrate
    MyPoints
    MySurvey
    Points2Shop
    Survey Junkie
    SurveySavvy
    SurveySpot
    Swagbucks
    Toluna QuickSurveys

    Most of the newbies in the beginning try to make money via paid surveys and PTC websites. You can search on google about these website and target them in your interests if they exist.

    Your offer is in IM niche but it is most likely suitable for newbies. Because of that, I suggest you to test targeting younger people like 16-30. You can also test targeting women or mom.

    Also, another important thing. Make sure that your landing page is mobile friendly. Now, about 80% of internet traffic come from mobile. Desktop traffic is 20% or even less than that.
