Can you embed a form on a Facebook page?

Hey Warriors, looking for some help, gladly pay you for info. I used to be able to embed forms on Facebook pages, but since recent changes, I can't figure it out easily. I need to embed a form on a page for an automotive client. Someone will fill out the info in the fields, hit submit, and info will go to an email address. Fairly simple, but not sure how it is done now, can anyone help? Let me know, MUCH appreciated!

I use JotForm.com on my facebook page. For the example: my official page - contact form.

Thanks for the help! Looks like Jotform is very Facebook friendly then, should work. Trying a lead gen campaign on Facebook combined with some targeted ads...we'll see what happens!
