Quick $50/$100 for eBook/graphic designers

I have an eBook I'm looking to put up on createspace.com (Amazon) It needs to be done to createspace specifications. You can find a specs here:

I already have the cover created in .psd format which I can supply. It simply needs the back cover portion added, created to the correct size (above) and the output furnished in .pdf format for uploading to createspace. I'll also need the raw .psd file as well.

If you can come up with/think of a better design that I approve, I'll pay you double. ($100)

Payment upon createspace acceptance. (probably same day submitted, I'll know (usually) as soon as I upload it. But if it comes back after manual review as unacceptable, it goes back to you. (These guys are PICKY...i.e do it right the first time.)
