Question about survey plugin / software

Hey guys. I'm in kind of a bind and I'm hoping someone here can help. I'm looking for a type of software / plugin that will allow me to create survey's like the one below...

Date A Different Girl Every Single Day

The problem is, I can't seem to find a solution anywhere. If you can provide a bit of insight, I'd appreciate it. if not, well, thanks for trying anyway

  Thanks for the feedback. It's not my website though, it's someone's Clickbank product that I found.
  
  The problem is, that I've been coming across several different landing pages with the EXACT same survey type, but none of them seem to run on wordpress, so I have no idea how to figure out what they're using.
